Medical record fees are determined at the state level and you can be charged a chart processing fee and can be charged per page - even if the clinic is only pulling your notes electronically. A lot of clinics now offer access to a patient portal where you can access a portion of your record for free. Note that what’s available via a patient portal is not your entire legal medical record. If you want a full copy of your legal medical record you need to sign a consent to release your medical record and submit the consent form to the medical records department. Regardless if it’s “your” record, you still need to sign a consent.
