Actually, "belly" is quite appropriate if you have pain or other issues that come from your abdominal area. The stomach is an organ, not the whole region. So, let's say you're having abdominal cramping. Saying your belly hurts is perfectly acceptable.
Please don't do #20. That is insanely tacky. If a guest put in the time, effort, and money into attending your wedding and giving you a gift, the least you could do is address the envelope yourself.
