Cardiometabolic syndrome is not made up. People with central obesity develop maladaptive adipose cells. These adipose cells (the cells from mcdonalds) cause insulin insensitivity and various other things, such as stopping the release of adiponectin, which is protective of your endothelial cells. Your endothelial cells prevent your blood vessels from filling with fat (from mcdonalds). To summarize: obesity causes insulin insensitivity leading to Diabetes, which causes heart disease, and causes your blood vessels to fill with fat, which causes heart disease. So, most of your points are now moot.
