Every restaurant I've ever been in is quite happy to serve drinks with little or no ice if requested. It's not automatic, I agree, but how hard is it, really, to say "No ice, please."?
Bakeries are definitely a bit harder to find in the US than in many parts of Europe, but that doesn't mean that superior quality bread doesn't exist. It just takes a little effort to find. That packaged white stuff in supermarkets is awful, of course.
