This man either had abdominal surgery (old fashioned large incision cholescystectomy, perhaps?) or a wound, and it is healing by secondary intention - i.e. he developed an infection, the wound was debrided, and is now healing (with nice red granulation tissue) from the inside out. There are lots of people out there with this kind of thing.

80s. The only male attention I ever got at a bus stop were either creepy old guys, drunk or high panhandlers, or street thugs wanting my wallet. There are upper middle class white guys at bus stops at places like Streeterville in Chicago - but otherwise it's poor guys and unemployed. She probably got a colostomy (or ileostomy) shortly after birth. And had a suprapubic catheter placed to drain her bladder - if she had a bladder. If not she may have had shunts placed into her kidneys connected to an external bag to drain off urine. Or dialysis (I don't know details of her physiology).
