It means that you need to break the fragile glass encasing the hammer (with your fist or whatever) to reach the hammer which you use to break the harder glass (which you wouldn't be able to break easily) on the bus or train in emergencies. It's encased so 1. an alarm goes off when it's accessed, and 2. so stupid chavs aren't smashing old ladies faces with it.

Generally, there is a smaller hammer attached to these things. Their only purpose is to break the glass that houses the larger hammer and sets off the alarm. Anyone who has ever seen one of these would know that.
